In the first quarter of 2026, the Hermitage Care and Rehabilitation Center operated under the ownership of LP CR Holdings, LLC, according to the Centers for Medicare and Medicaid Services (CMS). Located in Daviess County, the facility has been managed by LP CR Holdings, LLC since Nov. 1, 2007.

Hermitage Care and Rehabilitation Center operates on a for-profit basis.

During that timeframe, the facility received an overall CMS rating of 5, which is higher than Kentucky’s state average for nursing homes of 3.0. The CMS ratings reflect performance in health inspections, staffing adequacy, and resident care quality.

Based on the data, Hermitage Care and Rehabilitation Center served an average of 87.9 residents per day in the first quarter of 2026, with a capacity of 92 beds. The nursing home is not affiliated with a hospital.

The center reported no fines or penalties during the first quarter of 2026.

Altogether, five for-profit nursing homes operated in Daviess County, Kentucky.

U.S. Census Bureau data shows the population of older adults is on the rise as younger demographics wane. From 2020 to 2024, those age 65 and older rose by 13%, while the number under age 18 dropped 1.7%. The older population in the U.S. has seen substantial growth since 1920, increasing from 4.9 million to more than 61 million by 2024.

A study released in 2021 found that nearly half of Americans 65 and older—about 28 million—may need long-term services and support, with projections that more than one-third could ultimately require nursing home care.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
