The CMS gave Fordsville Nursing and Rehabilitation Center an overall rating of 3 out of 5, matching the average score of 3 reported for Kentucky nursing homes.
In comparison, Signature Healthcare of Hartford Rehab & Wellness held the highest bed count in Ohio County for the quarter, with 110 beds available. Beaver Dam Nursing & Rehab Center, Inc. was third, offering 58 beds.
As of the end of the first quarter of 2026, Fordsville Nursing and Rehabilitation Center had not received any fines or penalties.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
